WRIT SERVICE

The writ service I will provide:

1. I will open a file with the Sheriff’s department for you 
2. I then will serve parties and file proof of service and writ back
to the Sheriff
3. Mail any documents that may need to be mailed

 
You will need to fill out request form along with the following:
 
Earnings Withholding Order:
1. Original signed Sheriff's Instructions
2. Employer's name and payroll department address
or alternative service address
3. Debtor's social security number, if known
4. A copy of your money Judgement
5. Original signed Application for Earnings Withholding Order  
6. Original Writ of Execution
Bank Levy:
1. Original signed Sheriff's Instructions
2. Banks name and address
3. Debtor's account number
4. Debtor's social security number, if known
5. A copy of your money Judgment
6. Original Writ of Execution
Third Party Levy:                                        
1. Original signed Sheriff's Instructions
2. The third party's name and address 
3. A copy of your money Judgement
4. Original Writ of Execution
 


*You should also fill Memorandum of Costs After Judgment, Acknowledgment of Credit, and Declaration of Accrued Interest 
to insure collections of fees for collection on writ
